ANP

Set up: The sessions will be based in Ormskirk and the role is to work with the Community Emergency Response Team and they need a ANP who can provide an urgent response to patients in crisis in their own homes for patients with chronic long term conditions including frailty.

Rate: £40/hr

Times: Ideally 10am-6pm but any hours would be helpful.

January: 18th, 19th, 22nd, 25th, 28th, 29th

February: 1st, 2nd, 3rd, 4th, 5th, 8th, 9th, 10th, 11th, 12th, 15th, 16th, 17th, 18th, 19th, 22nd, 23rd, 24th, 25th, 26th
